Evaluations
Evaluations consist of a parent/guardian interview, direct assessment, and written report. Submitting intake paperwork and any relevant questionnaires that may have been provided to you at least 24 hours prior to evaluation is extremely valuable in allowing your therapist to arrive fully prepared and streamline the evaluation process.
Therapy Sessions
Therapy sessions are 60 minutes. This includes 50 minutes of direct therapy with you and your child and 10 minutes for documentation. A parent or caregiver older than 18 years of age must be present at all times. Therapy sessions are provided in your child’s home/natural environment. Clinic based therapy is not currently offered. Some motivational toys, devices, and basic therapeutic equipment may be brought with the treating therapist for use during the session however, personal toys and household equipment which your child is familiar with and which you can better use for practice between therapy sessions will be the most advantageous.
Progress Reports
Progress reports are written at six month intervals on your child’s birthday and on their half birthday. The report is provided to your GGRC Case Manager for inclusion in your Individualized Family Service Plan (IFSP) after review with you, the parent/guardian.
Cancellation Policy
Regular attendance and participation by you and your child is key for benefiting from therapy. If cancellation is necessary, please notify your therapist via phone or email with at least 48 hours’ notice. Rescheduling of appointments will be offered as available. Cancellations with less than 48 hours’ notice will be considered “late”. A session will be considered a “no show” if A) the therapist arrives and you and/or your child are unavailable or B) you do not show up at an agreed upon meeting place without notice. Reasonable exceptions will be made for illness or family emergency. Repeated “late” cancellations or “no shows” may result in services being suspended with Move and Play LLC.
Holidays and Vacations
All federal holidays are observed (New Year’s Day, Martin Luther King Jr. Day, President’s Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day). Therapists may take additional vacation time. Therapy sessions that fall within their vacation time are not rescheduled.
If you have a planned vacation, please let us know as soon as possible so that we may adjust our schedule. We will make every effort to make up missed sessions; however, this is not always possible due to scheduling conflicts. If you are taking an extended absence from therapy (4 or more weeks), you will be asked to vacate your therapy appointment slot. You may call us when you return to resume sessions.
Sick Policy
In consideration of the medical needs of the children we serve, it is important that we make every effort to avoid spreading illness between households. Please contact your therapist via phone or email as soon as possible if your child or someone in your household has had a contagious illness, rash, fever, vomiting, or diarrhea within the last 24 hours. The session will be cancelled with no penalty.
We will not be able to provide in-person service if staff or clients have any of the following symptoms:
- Red, swollen, or encrusted eyelids; discharge from eye(s)
- Thick yellow or green nasal discharge
- Unexplained or contagious rash
- Fever within the past 24 hours
- Vomiting or diarrhea within the past 24 hours
- Any Symptoms of COVID-19:
- Congestion/Runny nose
- Chills/Fever (100 Fahrenheit or above)
- Nausea/Vomiting/Diarrhea
- Fatigue/Headache/Muscle aches
- Sore throat/Cough/Shortness of Breath/Difficulty breathing
- Loss of taste or smell
If a child or caregiver becomes ill during a therapy session, the session will be ended.
